GRAND OPENING OF “502 TRANSITIONS”

An adult 30 bed residential group home serving the west end of Louisville! we focus on behavior modification, addiction, mental illness and a promising transition from prison to the streets reentry. We have licensed peer support specialist, clinicians and psychiatrist on site and we have a gang of case managers to do what needs to be done for our people to succeed.
